Job Description :
To provide a high quality resettlement service, with effective liaison with tenants/service users, support staff, and external agencies.
Main Duties:
To work with in conjunction with project workers to support clients who have the potential for move on, and co-ordinate a resettlement plan in conjunction with their Support Plan and monitor progress against the plan.
To provide practical support during the move, including accessing Social Fund grant and
loans, ensuring all benefits claims are submitted, helping to access furniture and
equipment and help with setting up arrangements for utilities.
To draw up and implement an individual resettlement plan with each tenant in
accordance with the policies and procedures of the project.
Support tenants to access community resources to meet a range of needs, including social and leisure needs.
You will need to have at least 6 months experience in a similar role and hold an Enhanced CRB check
