A well regarded social housing provider is currently looking to secure an experience Support Advisor to join their accomplished team in providing a valuable service to their tenants during times of vulnerability.
The positions requires an adept candidate to assist them in providing support to clients over a defined 6 week period, to return home from hospital and ensure that they are able to live safely at home and maintain their quality of life whilst recovering.
The day to day role will involve following support plans and existing assessments to ensure that individual needs are met and ensure that cleaning is completed where necessary and food prepared regularly if needed. The successful applicant will be required to liaised with and potentially visit doctors and other support providers to ensure that prescriptions and appointments are kept to.
It is essential that all applicants show integrity and emotional maturity and hold a full Enhanced CRB Disclosure and access to their own car.
